The medication is intended to counteract arrhythmic and hypertensive phenomena, localization of negative symptoms in cardiac dysfunctions. Concor tablets have a selective effect on the nervous system and are characterized by a hypotensive effect.

The composition (active substance) of the drug

It is delivered in two options marked by color of a cover. The composition of Concor differs in the concentration of the active component.

Yellow tablets in the composition are:

  • 5 mg bisoprolol in the form of hemifumarate and fumarate, the proportion is two units to one;
  • stabilizing additives. The combination of starch, cellulose, magnesium stearate and calcium hydrogen phosphate;
  • soluble film coating based on macrogol, titanium dioxide, hypromellose and color.

Beige or orange tablets bear:

  • 10 mg bisoprolol in the form of hemifumarate and fumarate. The ratio is identical;
  • stabilizing additives;
  • film coating. Similar to additives, the composition is constant in both tablet versions, the concentration of individual compounds varies slightly.

Additives in the active core are neutral in action on the gastrointestinal tract, provide uniform dissolution of the drug and preservation of the main substance. The shell is intended for the preservation of the drug until it enters the body when taken orally.

The product is available in heart-shaped tablets with a central groove on convex lateral surfaces.

pharmachologic effect

Concor - selective beta1-blocker, indirectly affecting smooth muscle bundles in bronchial and vascular structures, normalizes metabolic processes in tissues by acting on beta-adrenergic receptors.

Bisoprolol selectively affects the sympathetic part of the nervous system, activates receptors that regulate the pressure in the circulatory system and triggers their natural reflexes. At the same time, the enzymatic synthesis of the renal system is inhibited, the volume of renin excretion is reduced, which reduces blood pressure.

Therapeutic effect of Concor:

  • fights hypertension, lowers blood pressure by reducing the volume of cardiac output;
  • equalizes the frequency of contractions of the heart muscle, counteracting arrhythmic manifestations;
  • restrains neurohormonal hyperactivity, relaxes smooth muscles in the vascular walls, expanding their lumen, which is important for hypertension and dysfunction of the circulatory system;
  • regulates oxygen metabolism while eliminating muscle overload and reducing pressure inside the coronary vessels, preventing myocardial hypoxia;
  • suppresses angina attacks caused by hypoxia in myocardial tissues with ischemic pathologies, general circulatory depression, spasmodic events;
  • normalizes stroke volume of the heart and vascular tissue tone.

The drug does not violate local immunity in the respiratory system and does not cause metabolic imbalance. The concorder maintains the stability of the contractile ability of the myocardium, stabilizing the heartbeat by slowing the flow of pulses in the nerves and without inhibiting the heart muscles.

The absorbability of the drug is ninety percent. The effect peaks at a three hour period, continuing up to a day, even with a single use, since the half-life of bisoprolol takes twelve hours.

What helps Concor

Indications for use:

  • coronary artery disease, narrowing of the vascular lumen or partial blockage of coronary vessels with the development of cardiac dysfunctions;
  • hypertension;
  • rehabilitation after a pre-infarction condition or myocardial infarction;
  • chronic ischemia;
  • permanent insufficiency of the cardiovascular system;
  • arrhythmic phenomena, vasospasm, arterial hypertension.

Suppression of hypertension reaches steady values over a two-week course. Permanent therapy with Concor reduces mortality by up to 50% among patients with cardiac pathologies or heart attacks.

Instructions for use and dosage

The drug is used orally, without grinding, once in the daily interval. Suppose taking with food, before meals or after. The preferred moment is morning. The medicine is washed down with a third glass of pure water.

The course lasts from two months to six months with a gradual cessation of use in order to avoid the negative consequences of abrupt cancellation.

It is recommended that you consult a cardiologist before using Concor.

Depending on the clinical picture, the regimen and dose are adjusted by the attending physician. It takes into account the patient's condition and heart rate.

Dosage selection in typical situations:

  • stable heart failure - Concor is prescribed as a component of complex therapy involving cardioglycosides, ACE inhibitors, and blockers. The tablets are divided for accurate dose selection: the initial one is half the tablet, with a positive result, a whole tablet is prescribed for 2.5 mg per day. According to the described principle, the dose is increased to 10 mg at two-week intervals;
  • hypertension, the situation is not exacerbated angina pectoris imply the appointment of a daily dose of 5 mg, acceptable increase to 10 mg.An emergency dose with an upcoming attack is allowed, but the daily dose should not exceed 20 mg.

Basic clarifications for chronic failure:

  1. The dosage increases only after the patient has confirmed the tolerance of Concor.
  2. Deterioration of well-being means a decrease in dosage.
  3. In a three-month interval, the patient measures blood pressure, heart rate, and cardiac pathology dynamics. This indicates the effectiveness of the treatment or the need to change therapy for adverse events.
  4. Inhibition of respiration and heart rhythm, a critical decrease in pressure implies a reduction in the dosage of medications that are part of the therapy simultaneously with Concor, including diuretics, ACE inhibitors and adrenergic blockers.

The tool counteracts the occurrence of angina attacks and in the long term relatively increases the physical endurance of the patient. Concor does not develop a painkiller effect with an onset of an attack, but reduces the negative symptoms.

During pregnancy and lactation

Adrenergic blockers affect the peripheral nervous system and indirectly stimulate the tone of the smooth muscles of the uterus, which threatens with late delivery or miscarriage. Bisoprolol reduces blood flow between the circulatory system of the embryo and the embryonic organ, which creates risks.

The drug is avoided for patients with pregnancy and nursing mothers due to the lack of comprehensive studies substantiating the safety of use in these situations.

In case of emergency and the prevalence of the benefits of the drug for the mother's body over the estimated risks to the fetus:

  • the drug is used in therapy, but the reception is canceled in the three-day interval before the planned birth. This prevents the theoretically probable neonatal asphyxia and disruption of the cardiovascular system of the child;
  • if it is not possible to cancel the therapy, they continue the course of Concor until childbirth, but the child is examined and observed by specialized specialists.

The active component of Concor enters the mammary glands and settles in milk, but the effect of the substance on infants remains unclear for medicine.

Can I drink alcohol while taking Concor

The interaction of alcohol with bisoprolol in the composition of the medication gives negative consequences for the body, and in the short term. The combination of Concor and spirits provokes orthostatic hypotension, a drop in blood pressure up to a loss of consciousness and collapse.

Drug interaction

There are risks of negative consequences while using Concor with other drugs or too short an interval between taking two medicines.

The results of the combination of Concor with other medicines:

  1. Anti-arrhythmias (Diltiazem and others) - a critical drop in pressure, slowing down the work of the heart. The combination of two drugs of the same purpose creates an excess effect, exacerbates arrhythmic events and heart failure, and leads to blockade of the atrioventricular node.
  2. Medicines against hypertension, calcium antagonists (Nifedipine, Verapamil) - the development of hypotensive symptoms, inhibition of the cardiovascular system.
  3. Drugs for headaches, ergotamine products - a violation of peripheral blood supply.
  4. Clonidine, cardioglycosides with plant components (digitalis, adonis), Methyldopa, Guangfacin, Reserpine - vascular obstruction, cardiac abnormalities, slowing of the rhythm of contractions.
  5. NSAIDs, xanthines, estrogen-based drugs, antitussive drugs and drops with adrenergic agonists, Rifampicin - leveling the therapeutic effect of bisoprolol.

Co-use of the drug with MAO inhibitors is contraindicated.

The patient should notify the attending physician about all simultaneously used drugs, including over-the-counter drugs.

Contraindications, side effects and overdose

Use by patients under the age of eighteen is not recommended due to insufficient knowledge of the effect of the drug on a growing body.

The list of contraindications:

  1. Heart failure, which turned into exacerbation or decompensation.
  2. Pheochromocytoma. A sophisticated treatment plan is required.
  3. Personal intolerance by the patient’s body to the active component or stabilizing additives.
  4. Cardiogenic shock.
  5. Severe hypotension, with a pressure below one hundred mmHg.
  6. Acidosis.
  7. Acute pulmonary pathology, asthmatic syndrome.
  8. Depressed heart rate, heart rate below sixty beats per minute.
  9. The defeat or pathology of the sinoatrial node.
  10. Raynaud's syndrome, obstructed peripheral blood supply.
  11. Damage to the atrioventricular node of the 2nd and 3rd degrees in the absence of a pacemaker.

Side effects at the initial stage of therapy are manifested in the form of dizziness, dyspepsia and intestinal disorders, headaches, a feeling of physical malaise, decreased performance, and insomnia. The effects are either leveled during the first two weeks of using Concor, or they do not occur.

Other side effects:

  • inflammatory processes in the sinuses;
  • disruption of the excretory system;
  • rashes on the skin, hyperemia or a feeling of itching;
  • a feeling of numbness in the limbs;
  • shortness of breath, cough with pulmonary pathologies;
  • nausea and vomiting;
  • occasionally - convulsions, conjunctivitis.

Caution is advised in the appointment of patients with severe hepatic and renal dysfunctions. The permissible dose for the daily period is up to 10 mg.

There is no complete data on the effectiveness of Concor in the treatment of heart failure in congenital heart abnormalities with difficulty in circulation, diabetes, cardiomyopathy.

The maximum acceptable dose is 20 mg of the drug per day. Exceeding the dosage creates the basis for bradycardia - a slowdown in the heart rate, which is dangerous for health.

Symptoms in case of overdose:

  • tachycardia and arrhythmia, slowing of heart contractions;
  • spasmodic events and depression of the respiratory organs;
  • change in the shade of the skin to cyanotic or yellow;
  • sweating
  • drop in blood pressure and inhibition of heartbeat.

After detecting an overdose, the patient’s stomach is washed, adsorbing drugs are used to remove excess drug from the body, and additional therapeutic measures are carried out if necessary.

Analogs of tablets

The direct structural substitute for Concor is Bisoprolol, available in an equivalent dosage. The difference is in the content of auxiliary additives, the effect remains identical.

The following analogues of Concor are delivered by pharmaceutical networks:
